HEALTH AND VISION INSURANCE
Employees who work at least 20 hours per week and have been employed for three months are eligible to enroll in the Hospital's group health and vision insurance plan. The employee will have 31 days from their eligibility date to enroll in the group plan or sign a waiver card. If the employee does not enroll in the group plan during the 31-day period, he/she will be considered a late entrant and must wait until the annual open enrollment to enroll. Employees must maintain employment of at least 40 hours per pay period to maintain insurance coverage. If an employee works less than 40 hours per pay period they may, at the discretion of Administration be required to pay the current invoiced premium for pay periods not meeting the 40-hour minimum employment requirement.
If an employee's status changes during the year, the following guidelines will apply:
- If a participating full time or part time employee takes a leave of absence, the employee is responsible for making arrangements for payment of all premiums during his/her leave of absence.
- If a casual part time employee's status changes to part time or full time, the employee may enroll in the group health insurance within the 31 day eligibility period; the employee is responsible for contacting the Human Resources Office to request enrollment.
- If an employee's status changes from PRN to part time or full time, and he/she wishes to enroll in the group health plan, the employee must complete a three-month waiting period before he/she is eligible to enroll in the plan.
- A retired employee may continue participation in the group health plan.
- Employees who do not enroll during the 31 day eligibility period will not be able to enroll until the open enrollment period with the following exceptions: (a) spouse's employment is terminated or is no longer eligible for coverage under employer's plan; (b) spouse's employer sponsored medical plan is terminated.
- Covered employees may enroll new dependents for (a) marriage, (b) birth of a child, or (c) adoption of a child within the 31-day eligibility period
GROUP HEALTH COVERAGE CONTINUATION
Continued group health coverage will be offered to the employee and/or the employee's family under the COBRA law.
LIFE INSURANCE
All eligible employees who have completed three months' employment are automatically enrolled in a $25,000 term life insurance policy. Policy value decreases at age 65 and in five-year increments thereafter.
Employees who work at least 20 hours per week may purchase additional life insurance, through payroll deduction, for themselves and their dependents.
TUITION ASSISTANCE
It is the intent of Lincoln County Medical Center to encourage employees to seek continuing professional growth and development through continuing education, professional affiliations, literature review, and formal course work. Consequently, a policy of tuition assistance has been established. The following conditions must be met in order for an employee to be eligible for tuition assistance:
- Full time and part time employees to .8 FTE in good standing with Lincoln County Medical Center for at least three months
- Department Head and Human Resources approval prior to the beginning of the course

Evidence of successful completion of course(s) with a grade "C" or better
- Courses must be job related or considered to be of benefit to Lincoln County Medical Center
- Graduate courses are reimbursable, but are considered a taxable fringe benefit by the IRS
- Reimbursement for CEU's and other non-credit courses will be determined on an individual basis
Employees may receive tuition assistance for up to 15 credit hours per calendar year. Tuition for approved courses will be reimbursed at a rate of $100.00 per credit hour not to exceed actual tuition expense or $1500 per calendar year. Tuition assistance forms may be obtained from the Human Resources Department.
