If you or a family member are on your way to recovery, but need a little extra care, the Swing Bed Program at Lincoln County Medical Center may be perfect for you. The Swing Bed Program at Lincoln County Medical Center is often covered by Medicare and Medicaid, as well as many private insurance plans. We’ll work with you and your physician to define your needs and make sure that you’re receiving the appropriate level of care to qualify for the suitable coverage.
If you have questions about insurance coverage, Lincoln County Medical Center has a social worker and utilization review coordinator available to assist you. Our social worker will also assist with discharge planning, personal resources and financial needs.
Our compassionate nurses and support personnel work under the supervision of a physician to determine the best treatment plan for each patient. Because your needs are special, your treatment plan will be developed with your specific needs in mind providing continuous monitoring and patient/family education. Your treatment plan may involve any of the following:
Physical Therapy
Physical therapists are available to help rehabilitate injuries, build strength, prevent falls, and assist in your recovery. Their focus is on helping you resume life activities, particularly walking, exercising, maintaining balance, and building strength and flexibility.
Occupational Therapy
Throughout your stay, occupational therapists will work with you to restore daily living skills and increase patient independence, including bathing, dressing and grooming, cooking and basic housekeeping.
Speech Therapy
A speech therapist assists with recovering your communication skills, including expressing thoughts, reading, and safe swallowing.
Nutrition Services
Optimal nutrition is important in recovery and continued health. A licensed registered dietitian will offer nutritional support,education, and customized diets/menus to meet your personal preference and dietary needs.
Diabetes Education
A licensed registered dietitian and certified diabetes educator is available for patients with a new diagnosis or those who have questions or concerns regarding their chronic diabetes.
Pharmacy
We have an on-staff pharmacist who will fill your prescriptions during your stay and work with your physician to assure your medication needs are met.
Pulmonology/Respiratory Therapy
A pulmonologist is on staff and available to address any issues you may have during your stay with us. Our on-staff respiratory therapist can provide
education about oxygen and other respiratory needs.
Licensed Clinician/Social Worker
Our social worker will work directly with you and your family to connect you to the services and resources you need, both during your stay with us and upon your return home.
