The patient has the right to considerate and respectful care. |

The patient has the right to obtain from his physician complete current information concerning his diagnosis, treatment and prognosis in terms he can reasonably be expected to understand and to collaborate with his physician to make decisions involving his care. |

The patient has the right to receive from his physician any information he needs to give informed consent before the start of any procedure or treatment. |
|
4.
|
The patient has the right to refuse treatment to the extent permitted by law, and to be informed of the medical consequences of his action. |

|
The patient has the right to expect that all communications and records pertaining to his care will be treated as confidential. |
|
6.
|
The patient has the right to have access to information contained in his or her clinical records within a reasonable time frame. |
|
7.
|
The patient has the right to every consideration for his privacy concerning his own medical care program. |
|
8.
|
The patient has the right to expect that, within its capacity, a hospital must make a reasonable response to his request for service. |
|
9.
|
The patient has the right to be advised if the hospital proposes to engage in human experimentation affecting his care or treatment. |
|
10.
|
The patient has the right to obtain information about any relationship of this hospital to other health-care and education institutions insofar as his care is concerned. |

11.
|
The patient has the right to expect reasonable continuity of care. |
|
12.
|
The patient has the right to effective pain management. |
|
13.
|
The patient has the right to examine and receive an explanation of his bill regardless of the source of payment. |
|
14.
|
The patient has the right to know what hospital rules and regulations apply to his conduct as patient. |
|
15.
|
The patient may refer complaints concerning patient safety and the quality of care to the Social Services Department (636-528-3417), the Risk Management Department (636-528-3253), or the Joint Commission Office of Quality Monitoring (1-800-994-6610). LCMC will contact the patient and/or family with information within a reasonable time frame. |

16.
|
The patient or the patient’s designated representative has the right to participate in the consideration of bio-ethical issues that arise in the care of the patient. |
|
17.
|
The patient has the right to formulate advance directives and living wills and copies of those documents retained in the facility’s records. |
|
18.
|
The patient has the right to receive care in a safe setting, free from verbal or physical abuse or harassment. |
|
19.
|
The patient has the right to be free from restraints and seclusions of any form used as a means of coercion, discipline, convenience or retaliation by staff. |
|
20.
|
Parents of neonatal, child and adolescent patients have the same rights as outlined above. |
|
21.
|
The legally appointed custodial parent’s choice will prevail when conflicts arise between parents concerning the care of the patient, unless the hospital or the attending physician believes the choice is not in the best interest of the patient and a court order is issued. When two custodial parents disagree about the care choices, the conflict will be referred to the Bio-Ethics Committee. |
GRIEVANCES
In accordance with Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973 and the Age Discrimination Act of 1975, it is Lincoln County Medical Center’s policy to admit patients and give services without regard to race, color, national origin, age, sex or handicap.
If you feel you have been discriminated against due to race, color, creed, national origin, age, sex, physical ability, or source of payment for your care, you have the right to issue a written complaint, which the hospital will fully investigate. In addition, you have the right to report a complaint to the Department of Social Services Office of Civil Rights, P.O. Box 1527, Jefferson City, MO 65102. You may call this office at 1-800-776-8014.
Patient grievances regarding any such matters should be referred to the Social Services Department, Barbara Rapp, MSW, LCSW, the hospital’s 504 Coordinator, at 636-528-3417.
A grievance may be filed with the Missouri Department of Health, Bureau of Health Facility Regulations, P.O. Box 570, Jefferson City, MO 65102. You may reach this agency by calling 573-751-6303.
PATIENT RESPONSIBILITIES
Because your participation in your care is important, you as a patient, have the following responsibilities:
•You have the responsibility to provide, to the best of your knowledge, accurate and complete information to your doctor and other hospital staff. This includes information about current and past sicknesses, hospitalizations, medications, description of pain, and pain relief and health risks such as smoking, alcohol use, etc.
•You have the responsibility to report unexpected changes in your condition to your provider and hospital staff. If you can not comply with your treatment, please tell your doctor. You have the responsibility to ask questions or request additional information if you have been told about your care or treatment plan. Once agreed upon plan of care is determined, you are responsible to follow the established treatment plan of care and interventions of pain relief. You are responsible for keeping appointments, and when unable to do so for any reason, to notify your doctor.
•You should report your pain to your physician and other hospital staff so that the appropriate pain management goals and therapies can be established. You have the responsibility to alert the hospital staff when your pain control needs are not being met.
•To be responsible for your own decisions if you refuse treatment or do not follow the doctor's instructions concerning the treatment plan.
•To facilitate the arrangements for appropriate payment to the hospital for the care and treatment provided to you, including providing information about any insurance you may have or other payment arrangements.
•To follow all hospital rules, such as the smoking policy, visiting hours, and noise levels. To follow all hospital policies and rules affecting your care and consult while in the hospital.
•The unlawful possession of illegal drugs, drug paraphernalia, and prescription medication not prescribed for you is prohibited.
•You also have the responsibility to treat hospital staff and other patients with respect and consideration, including not swearing or using physical violence against them.
